Care - Homemaker Program -
Respite Care
In-Home Services
Jefferson Council on Aging provides several in-home services. These
include home delivered meals (described under
Home Delivered Meals), Personal
Care, Homemaker, and Respite Care services.
The goal of these in home services is to assist persons 60 years of age
and older, who are frail (homebound) and otherwise isolated, to secure
and maintain independence in their own residences as long as
circumstances warrant, by facilitating a system of integrated services
which consist of one or more of the following: Homemaker, Personal
Care/Respite, National Family Caregiver Support Program Respite, Family
Caregiver Support Program Personal Care, Family Caregiver Support
Program Adult Day Care, Family Caregiver Support Program Material Aid,
Family Caregiver Support Program Individual Counseling, and Referrals
to other resources for caregivers or elderly customer.
Because the demand for in home services is so great, there are waiting
lists for most of these services.
Paperwork:
A note about forms and information. To obtain in-home services or
continue to receive any of our services, the Jefferson Council on Aging
must comply with local, state and federal requirements and individual
program guidelines for obtaining and verifying information. This may
mean a visit to your home or a call to you or your caregiver from a
representative of the Council on Aging to complete an assessment form.
Your help and cooperation will help us to serve you better. None of our
services are entitlement programs.
Personal Care:
The goal of the Personal Care Program is to provide personal care
service for seniors 60+ who are confined to their home and unable to
attend to their personal care. This service is subcontracted with a
licensed agency.

A needs assessment worker will come to the home to complete an
assessment on the senior.

A care plan is developed for each client that will help him/her to
remain independent in his or her home.

The Personal Care attendant goes to the client's home as scheduled
for a two-hour period to bathe and perform other personal hygiene
according to the care plan.

A yearly reassessment of the needs of the client is conducted and the
care plan is revised accordingly.

The Personal Care attendant receives 40 hours of training each year
and the program is certified through the state.
Homemaker Program:
The Homemaker Program's goal is to provide an array of in-home
housekeeping services for the elderly who are frail, homebound by
reason of illness or incapacitating disability. This is a subcontracted
service.

A homemaker goes to the homes of eligible clients as scheduled for a
one to two-hour period to perform light housekeeping tasks.

Clients must be living alone or with another senior.

These activities are documented by the homemaker's weekly worksheet.
A yearly reassessment of the needs of the client and the performance of
the homemaker is conducted.
Respite Care:
The goal of the Respite Care Program is to provide respite care service
for caregivers who are caring for seniors age 60+ who are confined to
their home and unable to attend to their personal care. This service
allows the caregiver the opportunity to have a well-deserved break from
care giving responsibilities. See National Family Caregiver Support
Program. This is a subcontracted service.
